Warning, Waiver, Release of Liability & Agreement to Participate

IMPORTANT – PLEASE READ CAREFULLY BEFORE SIGNING

This Agreement must be signed by all participants. If the participant is under 18 years of age, it must also be signed by a parent or legal guardian.

Acknowledgment & Assumption of Risk

In consideration for being allowed to participate in classes, training, events, or activities offered by JSK 5.0 Montebello, MG Martial Arts and/or MG Muay Thai (“the Business”), I agree as follows:

• I will inspect all equipment, mats, and facilities prior to participation. If I observe any unsafe condition, I will immediately notify an instructor and refuse to participate until the issue is corrected.

• I understand that martial arts and combat sports involve physical contact and strenuous activity, and I fully acknowledge the inherent risks, including but not limited to:

  • Serious bodily injury

  • Permanent disability

  • Death

  • Emotional, social, and economic loss

• I understand that such risks may arise from:

  • My own actions or negligence

  • The actions or negligence of others

  • The condition of the premises or equipment

  • Risks that are unknown or not reasonably foreseeable

• I voluntarily assume all risks associated with participation and accept full personal responsibility for any injury, damage, or loss that may occur.

Voluntary Participation & Physical Condition

• I am participating voluntarily and of my own free will.

• I agree to follow all instructions given by instructors and staff at all times.

• I certify that I am in good physical condition and do not suffer from any condition that would prevent safe participation.

• I understand it is my responsibility to consult a physician prior to participation if I have any medical concerns.

Medical Authorization

I authorize the Business to obtain emergency medical treatment for me if necessary, including care from physicians, nurses, or emergency personnel. I understand that I am financially responsible for any medical treatment provided.

Release of Liability

I hereby release, waive, discharge, and agree not to sue the following parties:

• Maurice Gomez

• JSK 5.0 Montebello

• MG Martial Arts / MG Muay Thai

• Jeff Speakman’s Kenpo 5.0

• All instructors, staff, volunteers, and affiliated organizations

• Event organizers, sponsors, advertisers

• Property owners and lessors

(collectively referred to as “Released Parties”)

From any and all claims, liabilities, damages, or losses, including those resulting from negligence, related to my participation in any class, training, or event.

Parental/Guardian Consent (For Minors)

I, the undersigned parent or legal guardian, acknowledge that:

• I have reviewed and explained this Agreement to the minor participant.

• I consent to the minor’s participation and accept all risks on their behalf.

• I agree to all terms, waivers, and releases stated above.

Conduct Acknowledgment

I understand that respectful behavior is required at all times. I acknowledge that if my behavior (or my child’s behavior) is deemed unsafe, disruptive, or inappropriate, JSK 5.0 Montebello reserves the right to suspend or terminate participation immediately without refund.

Acknowledgment of Understanding

I/We have read this Agreement in full and understand that by signing it, we are giving up substantial legal rights, including the right to sue.

I/We sign this Agreement voluntarily and without coercion.
